Your online patient questionnaire history is still available for you to access via our previous website.

Please click here to access your patient questionnaires.

Otherwise, to send a request to a patient to complete one of our online questionnaires, go to the MY CLINIC tab in the main menu.

Go to My Patients to send a request to an existing patient or Add New Patient to invite a patient.

Once a patient is created you can send them a request to complete one of the online questionnaires.
